Our Home and Community Care services offer support to older Australians who choose to live independently in their own home. Our staff are friendly and fully qualified, and will spend dedicated time to getting to know you one-on-one, in order to fully understand your needs.

How Can We Help?

When you choose our Home and Community Care services, we will organise a time to visit you at home. This will allow us to have a thorough understanding of what kind of support you need, so that we can create a plan to meet your needs. Our services support people living at home with tasks such as cleaning, cooking, transport and nursing care.

Bathing and showering, dressing, grooming, meal preparation, eating and drinking.

Washing and ironing, cleaning, laundry and shopping.

Clinical monitoring services, specialised nursing care such as wound management, prompting or assistance with medications and other services as needed.

Assistance with using mobility aids and equipment, encouragement to go for walks or to exercise.

Support with minor repairs and maintenance of your home or yard such as lawn mowing and changing light bulbs.

Physiotherapy, podiatry, speech pathology, occupational therapy, and dietician.

Please contact our Client Services team on 1300 111 227 and ask to speak to a Home & Community Care representative.

We endeavour to respond to all client queries within 24 hours.

In order to transfer your package, you first need to agree in writing to receive services from us and confirm your required start. Following this, you would then notify your current provider in writing that you are transferring to our services. Any unspent monies will be transferred from your previous provider to your new provider within 56 days. We will also cover any exit fees that your previous provider charges.

Please contact your assigned Home & Community Care Coordinator who will be more than happy to assist you with a simple re-assessment and guide you through the process.

Please keep in regular contact with your Home & Community Care Coordinator who is there to advocate on your behalf and provide you support.
