Together, we’ll create a care plan that suits your life. And as life changes (as it often does), your plan can change too.

Support with things like nursing, medication, monitoring your health, and day-to-day personal assistance such as showering, dressing, getting ready for the day - done respectfully and at your pace.
Help with cleaning, laundry, and keeping your home clean and comfortable.
Someone to chat with, head out with, or simply help you stay connected to the people and routines you enjoy.
Support with grocery shopping, meal planning, and preparing food at home.
Help with transport for appointments, errands, or getting out and about.
Access to Allied Health services like physiotherapists, occupational therapists, speech pathologists, dietitians, psychologists, audiologists, pharmacists and podiatrists.
Help arrange equipment and supplies to support your safety and comfort, e.g. walking aids, wheelchairs, shower chairs etc.
Help with gardening, small home maintenance jobs and modifications to make it easier for you to live independently e.g. installing grab rails, ramps, non-slip flooring etc.
Guide through your home care options, paperwork, and funding.

There are many ways to fund in-home care and support, but for many people, trying to work out what is right for them can be difficult and confusing. We can help.
We can also assist with applying for additional supplements such as dementia and cognition, financially disadvantaged and socially disadvantaged supplements.

If you are an existing client, you can contact your dedicated care partner with any queries.
We respond to all client queries within 24 hours with the exception of weekends and public holidays where we will respond on the next business day.
In order to transfer your Support at Home program, you first need to agree in writing to receive services from us and confirm your required start. Following this, you would then notify your current provider in writing that you are transferring to our services. Any unspent money will be transferred from your previous provider to your new provider within 56 days. During this 56 day period, you can opt to start receiving service from us by negotiation and as agreed in writing.

We have no establishment or set up fees, and no exit fees.
