Helping older people live at home for longer, in safety and comfort
Care Partners manage and coordinate the delivery of the range of services that are provided to our Salvos Home Care clients, working in partnership with clients, families, Home Care Workers and the broader Salvos Home Care team.
As a Care Partner, you will monitor clients’ wellbeing, identify clinical needs and help with interventions to ensure care remains safe, responsive and centred on the individual. You will also provide clinical guidance and mentorship to our Home Care Workers, supporting consistent, high-quality care across the team.
With support from the Salvos Home Care Team Leader and our broader Aged Care Support Services team, this role combines clinical expertise, care coordination and meaningful relationship-building to promote each client’s independence, wellness and lifestyle choices.
Home Care Workers - Domestic Assistance provide services to help our clients continue living at home. In this role, you will assist clients with general household tasks and cleaning based on their individual needs and preferences. As a Home Care Worker, you will build respectful and trusting relationships with clients, their families and supporters, and follow the tasks as directed as part of the Care Plan. Working with the Care Partner, you will monitor the personal needs of clients and assist them in performing tasks as needed and report any changes relating to a client's care needs.
