We understand how important it is to find the right home for you or your loved one. To help you navigate through the process of accessing residential aged care, we’ve created the following five-step guide. If you’d like us to walk through this process with you, we’re here to help, every step of the way.

1. Obtain care approval from My Aged Care.

My Aged Care is the main entry point to all government-subsidised aged care services in Australia. The steps are the same whether you’re applying for permanent care or respite (short-term) care. 

  • Check your eligibility. 
  • If eligible, contact My Aged Care on 1800 200 422 or visit myagedcare.gov.au to apply for an aged care assessment.  
  • If the application is successful, you will be referred for an assessment. Assessments are done in person, usually at your own home. The assessor will decide your care needs and approval for entry into subsidised residential aged care.  
  • Once approved, get in touch with us on 1300 111 227. 

2. Contact us and take a tour of your preferred aged care centre.

Deciding which aged care centre is best for you is an important and personal decision. We recommend that you visit your preferred aged care centre so that you can experience the loving care we provide and meet the dedicated team of carers who will work with you to ensure your care needs are met. Contact us and take a tour of your preferred aged care centre. 

At the conclusion of the tour we will go through the information pack and explain the fees and charges.

3. What you'll need to apply.

To apply for residential aged care, please provide the following:

  • Completed application form
  • Aged care assessment or support plan
    Provided by the government assessment team.
  • Combined Assets and Income Assessment letter
    Issued by Centrelink or the Department of Veterans’ Affairs - required if you’d like further assistance from the government with what you have to contribute towards your care.
  • Certified Copy of Power of Attorney, Enduring Power of Attorney or Guardianship
    If applicable.

4. Meet with the clinical team. 

Once we have received your documents and have a suitable vacancy, we will arrange a pre-admission interview so that we can discuss your care needs and preferences in more detail.

5. Move into your new home!

Once we’ve confirmed that we can meet your care needs and have a suitable room available, we’ll offer you a place and arrange a move-in date that works for you.
